Registering With The Practice

PreRegistrationIf you wish to register with the Practice you will need to collect the registration form along with a new patient health questionnaire form from Reception. 

When you register you will also be asked to fill out a health questionnaire. This is because it can take a considerable time for us to receive your medical records. This form is scanned into your medical record.

Patients aged 16+ years will need to provide 2 forms of identification when bringing in their registration form: 1 photographic (for example, passport or driving licence) and another with name and new address. If photographic identification cannot be produced then 2 forms of name and new address is required.

Alternatively you may print off a registration form and patient health questionnaire, fill the form out and bring them in with you on your first visit to the practice. If you are aged 18 years+ you will also need to bring with you 2 forms of ID: 1 photographic and 1 with your new address details (we require to see each person with their ID unless unable to do so due to certain medical factors e.g. physically housebound)
